- HSA provides a 1:1 match ratio up to a maximum amountof $500 with no original donation minimum.
- Choose from list of non-profit or disaster relief fund or designate the program of your choice with 501 (c) (3) status.
- Deadline for submission is September 1 of calendar year.
Upon receipt of this form your initial donation will be verified and HSA will match your donation up to $500.
